I'll take you through step by step as you do it, that way confusion can be cleared up quickly if it occurs.
First, sign in. Click on the 'Publish' slide-out menu, then on 'Doc Manager (Upload)'. From there you can label your document (don't forget to select one from your computer to upload). Click 'Submit document'; it should appear up in the table above the form you filled out previously.
