I've recently fallen victim to 'wrongly' assuming someone to be straight and they were very offended. So what I want to know is why it is offensive to some people to make what is usually an honest mistake.

Because of the person.
Some people take things like that very lightly, while others take it to heart. It really depends on the personality and ways of the person. -
Because they are insecure about it.
