It's a uniquely generated image of a cat that doesn't exist, assembled by an Artificial Intelligence that is fed images of real cats in order to construct fake cat pictures.
You can tell because sometimes the details will be off, or you can look in the background and see it's glitchy or wonky, or random cat fur textures will be strewn about. -

So normally when you post an image with the UBB code, you are posting the URL with an extension to that specific picture. However, thiscatdoesnotexist.com does not have unique extensions for each cat picture. It's just the site itself. Also, the website doesn't have any text, only an image. So you can post it in the UBB code without having to open the photo in a new tab or anything. You just post the URL.
However, because every time you refresh the webpage it gives you a different photo, the same happens here. There's no extension tying it to a single photo. It works just like the site.
