I think I understand your question...I think.

Physical pain is not the only kind of pain.

And plus, nobody really knows what heaven and hell are like, to be honest, other than the fact that heaven rocks and hell sucks. Like, alot.
And hey - what if your mind can think you're being hurt, causing you to think you're in pain, so you feel pain?

Considering the fact the mind thing is triggered by pain receptors in the brain, a physical thing, I don't think that's how it works.

It's more along the lines of a deep guilt for what you have done.
I always viewed this as more of a "karma bites you in the ass" stage.
