This is good. At least I liked it. But you REALLY need to work on your spellings. And another point, some options need to be there for the takers instead of some unnecessary options. For e.g, I may not think my child inappropriate for group or one-on-one dating even when she is 17-18! So this option needs to be there! Otherwise okay, keep it up.
